Here you will find the benefit summaries for all of the EmblemHealth Medicare Advantage HMO plans for 2014 or 2015 in New York City and Nassau county. The benefit summaries are a basic view of the benefits. Please call or write to request the enrollment kit. The enrollment kit includes the full benefit summary and the enrollment forms for 2014 or 2015. Annual open enrollment for medicare is from October 15 thru December 7 2014. All plans become effective January 1 2015. There is continuous enrollment for any low income beneficiaries and also new to medicare beneficiaries. There are also several special enrollment options for specific circumstances, call me for more information.

EmblemHealth Medicare Advantage HMO plans are low cost with a great local network of providers. All plans include part D drug coverage. EmblemHealth HIP HMO plans have a very large network of providers in NY city and Long Island. $0 deductible part D prescription plans are included. Many supplemental benefits are included. Basic dental and vision and Gym membership discounts are included.
