2. Blue Shield of California - Is a California based health insurance company.  Blue Shield offers HMO and PPO plans with 3 different HMO networks and the full PPO network.  1099 employees are not allowed.  Blue Shiled will require 65% of your employees to enroll after valid waivers are considered.  Valid waivers are Other group coverage, Medicare, Medi-cal, spousal, and TRICARE.  Blue Shield will allow up to 49% of your employees to be covered out of state.   The Blue Shield PPO full network is the 3rd largest network of providers in the Los Angeles area.  Provider search  Blue Shield plan guide for small businesses

3. CaliforniaChoice - Is a California private Health Insurance Exchange.  With Calchoice you can offer your employees up to 7 different health insurance carriers ( Anthem Blue Cross, Healthnet, Kaiser Permanente, Sharp, SutterHealth, Unitedhealthcare and Western Health Advantage) depending on what county your business is located in.  With Calchoice you can provide many different health insurance choices for any size group.  Just offer your employees a fixed percentage contribution and let the employee pick the health plan that is right for them.  Under this system your business will stay on budget and your employees will have the insurance that meets their needs  With CaliforniaChoice your small business can offer large company style benefit choices no matter how many employees you have.  Calchoice will require 70% of your employees to enroll after valid waivers are considered.  Valid Waivers are spousal, Medicare, Medi-Cal,and Tricare.  1099 employees are not allowed.  Calchoice will allow up to 49% of your employees to be out of state,  the out of state employees will be offered an Anthem Blue Cross PPO plan. 4. Anthem Blue Cross - is a national Blue Cross provider.  Anthem will offers 2 HMO networks and 2 PPO networks to choose from.  Anthem will require 70% of your employees to enroll if your business employs between 1 -14 employees.   If you employee 15 or more full time employees Anthem will require 50% of your employees to enroll after valid waivers are considered.  Valid waivers are individual coverage, Spousal, Medicare, Medi-cal and tricare.  1099 employees are not allowed.   Anthem will allow up to 49% of your employees to reside out of state.  The Anthem PPO full network has the largest provider network in the Los Angeles area.

5. Aetna-  Is one of the largest national insurance companies.  Aetna offers 4 different HMO network plans and 4 PPO network plans.   Aetna will require 60% of your employees to enroll after Valid Waivers are considered.  Valid waivers are Spousal, other group coverage, Medicare, Medi-Cal and TRICARE.   1099 employees are not allowed.   Aetna will allow up to 49% of your employees to be out of state.  Aetna provider search  Aetna plan guide for small businesses

6. HealthNet - is a large national health insurance company. -  Healthnet offers up to 8 different plans, HMO, EPO and PPO plans are available with a few different network options.  Healthnet will require 66% of your employees to enroll after valid waivers are considered.  If you employ between 6 - 100 employees the participation drops down to 50% after valid waivers are considered.  1099 employees are not allowed.  Valid waivers are spousal, Medicare- Medi-Cal and Tricare.  Healthnet will allow for up to 49% of your employees to live out of state.  Provider search

7. Kaiser Permanente - is a large regional health insurance company.  Kaiser offers an HMO plan and a PPO plan.   Kaiser requires a minimum of a 50% contribution towards Employee coverage and Kaiser requires at least 70% of your employees to enroll after Valid waivers are considered.  1099 employees are not allowed.  Kaiser will allow for up to 49% of your employees to reside out of state.  Valid waivers are Spousal, Medicare, Medi-Cal and TRICARE.  Kaiser owns and operates their hospitals and facilities. Also Kaiser employs their providers.  With this model of healthcare there can be a better level of care, since the providers don't have an incentive to bill for service.
